A caregiver in this position works days,8:00AM to5:00PM.
A caregiver who excels in this role will:
Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional,financialand environmental problems.
Perform psychosocial assessments and provide interventions toassistpatients and families. This may include counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problemsolvingand coordinating family/team meetings.
Provide and/or document treatment recommendations, professional recommendations, care coordinationinterventionsand treatment plans.
Engage patients and families in adhering to the treatment plan by reducing oreliminatingbarriers.
Maintain a network of resources for consultation and connect patients with community resources.
Supervise social work interns andassistwith training new caregivers.
Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.
Minimum qualifications for the ideal future caregiver include:
Master’s Degree in Social Work from a CSWE accredited program
State Licensed Social Worker (LSW)
Two years of experience in a hospital or healthcare setting
Licensed Independent Social Worker (LISW) within five years of hire
