Indian River Hospital’s Case Management department is looking to add a Social Worker to the team, who willassistin discharge planning for patients. In this role, you willcoordinate social work services for Med/Surg, ICU, Telemetry and Emergency Room patients and provide psychosocial assessment, counseling, resource information, and referrals to these patients and their families.You will develop skills to cope with the impact of illness, hospitalization, treatment, and continuing care issues, functioning as the patient’ssafety advocate. APRNcaregiver in this position works weekend days from 8:00AM – 4:30PM. Someweekdayswill berequired.
A caregiver who excels in this role will:
Identifysignificant psychosocial issues andimplementappropriate socialservice interventions and continuing care options through the process of data collection, dataanalysisand implementation of a care plan based onidentifiedneeds.
Demonstrate the knowledge and ability to care for age specific needs of the population served as measured by completion of mandatory training and direct observation by supervisor.
Demonstrate an understanding of illness/recovery and the impact on patients and families.
Respond to campus widereferrals on thesame working day.
Utilize a multidisciplinary approach to ensure continuity of care and comprehensive patient services to maximize the effectiveness of social service interventions.
Develop and sustain positive workingrelationshipswith patients, families, physicians,nursesand ancillary personnel.
Act as a resource to patients, families,physiciansand staffregardinginternal/external resources.
Initiate team conferences tofacilitatediscussion/consensusregardingcomplex cases.
Usecrisis intervention techniques to calm,comfortand stabilizepatient/families under stress.
Utilize knowledge to coordinate realistic referrals and continuing care plans to meet the needs,ageand health status of the patient.
Inform the patients,familiesand anyone accountable for the continuing care of the patient of their role and responsibilities relating to the continuing care plan.
Utilizeappropriate clinicalintervention skills toassistpatient,familiesand staff with their abilities tocope.
Document on CM progress notes, multidisciplinary problemlistand interdisciplinary education records to provide clear, conciserecordof social work services and interventions.
Facilitate and coordinatetimelytransfer and discharge of patients with complex needs and inpatient and outpatient dialysis arrangements.
Disclose medical errors to patients and familiesin accordance withexisting patient safetyplanand disclosure policy.
Assistin developing andmaintainingthe electronic case management resource manual.
Act as a campus wide resourceregardingthe Baker Act.
Minimum qualifications for the ideal future caregiver include:
Bachelor's Degree in Social Work(BSW)
Two years of experience as a social worker in healthcaresetting
Excellent oral and written communication skills
Ability to prioritize and multitask to achieve a therapeutic outcome
