Cleveland Clinic’s Home Care team helps patients continue their treatment and learn/relearn any skills necessary to stay safe and independent again. As an RN, youare responsible forproviding direct nursing care,participatingin a team-based care model, implementing and monitoring patient care plans, and reporting patients' needs after every visit.Due to continuously working with each patient, Homecare RN’s have more acute findings for patients and are easily able toidentifychanges.Come bea part of a team where you can make a meaningful impactforpatients and their families when they need it most.
A caregiver in this position works days from 8:00AM – 5:00PMor 8:00AM – 6:00PM, withon-call requirements.
A caregiver who excels in this role will:
Provide direct nursing carein accordance withestablished policies,proceduresand protocols of the healthcare organization.
Implement andmonitorpatient care plans.
Provide supervision to LPNs, home care aides,familyand caregivers involved in the care of the patient.
Performaccurateand complete admission assessments and reassessments withinspecifiedtime frame.
Assess patients for safety risks,abuseand neglect.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Assess and document patients' pain usingappropriatepainscale and manage patients'pain.
Prioritize and document patient problems based on assessed needs and problems in collaboration withpatient,familyand health careteam.
Maintain complete and up-to-date documentation and files at all timesregardingpatient condition and effectiveness and timeliness of care and interventions.
Escalate change in patient condition to patient’s physician asappropriate.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited school of Professional Nursing(Diploma,ADNor BSN program)
Current state licensure as a Registered Nurse (RN)
Basic Life Support (BLS) Certification through the American Heart Association (AHA)or the American Red Cross
Valid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafter
Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter
One year of currentclinicalexperience as an RN
One year of experience in an acute Medical/Surgical or Home Care setting
