A caregiver in the ClinicalPharmacist – Cardiology role supports the Heart, Vascular & Thoracic Institute (HVTI) at Main Campus.This role will include caringcomplex, high acuitycardiologypatientsacross many subspecialties.This is a hybrid centralized /decentralizedrolewithrotating responsibilitieson cardiology subspecialties and operations.
A caregiver in the Pharmacy Clinical Specialist - Cardiology role will include responsibilites outlined above in addition to the opportunity to collaborate in morequality,research and committee/leadership activities but will have the same core patient care responsibilitiesfor patient care and educationas a Clinical Pharmacist.
A caregiver in this role will work a variety of shifts between 6:30am - 10pm.

Patient Care
Combine clinical and operational knowledge tooptimizemedication therapy.
Prospectively review,interpretandclarify, medication orders to identify, prevent and resolve potential and actual drug related problems.
Provide evidence-based recommendations to providersand recommend formulary alternatives when appropriate.
Serve as the medication expert foroptimizingdrug therapy
Education
Precept and mentor pharmacy residents and students through experiential education experiences
Teachandguide pharmacy and non-pharmacycaregivers,residentsandstudentsthrough targeted formal and informal education

Identifyand escalate clinical and operationalissuesimpactingpatient care or workflow.

Minimum qualifications
PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy
PGY2 residency or fellowship trainingpreferred;or
PGY1 residency plus minimum of 2 years of relevant specialty experienceas aclinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area;or
Minimum of 4 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care
