Position Summary
Healthcare professional with experience in care coordination, chronic disease management, and interdisciplinary collaboration across the continuum of care. Skilled in case management, patient flow optimization, community and primary care coordination, patient education, and evidence-based care delivery to support improved patient outcomes and healthcare resource utilization.
Duties
- Coordinate referrals from acute care or the community for appropriate admission to West Park and/or link with appropriate services in the community as part of integrated management of chronic respiratory disease
- Provide intensive case management for patients with Neurological conditions across the continuum of care
- Act as the point of contact/liaison between members of the collaborative care team
- Promote program efficiencies in patient flow and healthcare resource utilization
- Coordinate, monitor, and facilitate patient’s care plan; emphasizing early engagement of primary care and involving community partners, as appropriate
- Organize systematic case reviews with the collaborative care team
- Provide education to patient/family; coordinate development of self-management plan
- Assist patients in navigating the healthcare system
- Ensure delivery of evidence-based (best practice) patient management
- Ensure baseline and follow-up evidence-based outcome measures are conducted
- Ensure collection of system-based metrics (i.e. volumes, wait time, health care utilization)
- Ensure collection of clinical outcome metrics at the program level
- Perform other duties as assigned.
