Job Summary

Nurse Coordinator,  Autoimmune Liver Disease (AILD) plays an important role in supporting patients living with autoimmune liver diseases, such as autoimmune hepatitis, primary biliary cholangitis, and primary sclerosing cholangitis. This role focuses on coordinating care, educating patients and families, and helping patients navigate often complex, long-term treatment plans. This is a great opportunity to make a meaningful impact in hepatology by improving patient outcomes and supporting individuals through every stage of their care.

We are looking for someone with strong clinical knowledge, excellent communication skills, and a genuine passion for patient care.

Duties

• Educate patients and their families about their condition, medications (including immunosuppressive therapy), and what to expect over time
• Support patients in managing their treatment plans and monitoring for side effects
• Help connect patients with resources and support services
• Keep clear and up-to-date documentation of patient interactions and care plans in EPIC
• Work closely with lead hepatologist and the broader care team to ensure smooth, coordinated care
• Take part in quality improvement initiatives and program development
• Contribute to the academic mission of the autoimmune liver disease clinic, by helping patients understand the importance of, and participation in, approved research studies
