This is a unique opportunity to be part of a tight-knit collaborative team, you will assist palliative patients with personal care and activities of daily living. You will support patients, families and caregivers –enhancing quality of life, comfort, dignity and security in patients’ end of life journey. All direct patient care activities will be performed under the direction of an Integrated Care Program Supervisor who is a nurse.
- Working alongside the interdisciplinary palliative care team, you will provide specialized care as specified in the care plan, according to protocols, within scope of practice and in accordance with Spectrum Health Care’s policies and procedures.
- Provide companionship and comfort, creating a calm and respectful environment tailored to the client's needs, preferences, and goals of care.
- Assist person(s) with activities of daily living such as bathing, toileting, hygiene, skin care, light meal preparation, feeding and transfers.
- Perform light housekeeping duties such as decluttering and cleaning
- Listen, observe and communicate any concerns/changes in the patient’s physical and/or emotional condition to the Integrated Care Program Supervisor.
- Travel within assigned region between patients’ residences or facilities.
