Affiliated Pediatric Practices (APP) is a network of independently owned Massachusetts-based community pediatric practices. Our goal is to offer excellence in healthcare to our pediatric patients and families by combining the highest-quality care with APPās outstanding efforts and dedication toward providing exceptional patient experiences. With over 30 years of experience, APPās pediatricians collaborate to share knowledge and implement innovative programs, improving patient care and experience across Massachusetts. Today, APP has more than 100 physicians, 45 nurse practitioners, and 15 behavioral health providers in 32 offices throughout Massachusetts. This unique collaboration, along with our partnership with Mass General Brigham has enhanced our ability to exceed national pediatric healthcare standards. Job Summary
We are seeking a compassionate, full-time, 40-hour Community Health Worker (CHW) to join our APP team! This hybrid role offers a flexible schedule, ideally Monday ā Friday, 8 am ā 5 pm, with 4 days on-site and 1 day remote. While primarily supporting from our Dedham office 2 days per week, the CHW will also be embedded 1-2 days per week at APP practices within a reasonable distance to provide in-person support.
The CHW will work closely with patients, families, and community resource agencies to promote and maintain good health. This includes providing education, support, and outreach services to help community members achieve their health goals. Areas of need most commonly include patient access to resources such as food, housing, childcare, and financial support; may also include behavioral health/substance use/medication management, transportation, job skills training, healthcare system navigation. The CHW works under the direction of a healthcare professional to deliver preventive care and health education to the community.

- Associates Degree or Bachelor's Degree Related Field of Study preferred
- Experience in community outreach, health education, doctor's office, or related field 0-1 year preferred
Knowledge, Skills, and Abilities
- Bilingual skills preferred; Languages in order of greatest demand: Cape Verdean Creole, Brazilian Portuguese, Spanish, Haitian Creole
- Strong communication and interpersonal skills, with the ability to interact effectively with diverse populations.
- Demonstrated ability to work effectively and provide advocacy for racially and ethnically diverse populations and communities preferred.
- Ability to work independently and as part of a team.
- Basic computer skills, including Microsoft Office and database management.
