Position Summary
The Speech-Language Pathologist is an integral member of our Neurovascular Unit providing expert assessment and treatment for adults with communication and swallowing disorders following stroke. The successful candidate will work in a progressive multi-professional team, participate in rounds, meetings and quality initiatives and assist with coverage across other clinical service areas as necessary. The candidate will perform all other duties as required. The successful candidate will also fill the role of Stroke Outreach SLP. In this capacity they will provide consultative assessment in the Stroke Prevention Clinic (SPC) and TIA and Minor Stroke Unit (TAMS) as well as support stroke education and participate in initiative development to optimize care for persons with stroke across the Toronto Stroke Network.
Duties:
Inpatient Neurovascular Unit (NVU) 0.5 FTE
- Conduct comprehensive communication, cognitive鈥慶ommunication, and swallowing (dysphagia) assessments for adults with stroke and neurovascular conditions.
- Perform timely bedside swallowing evaluations and determine safe oral intake.
- Identify aphasia, dysarthria, apraxia of speech, and cognitive鈥憀inguistic impairments related to acute stroke.
- Develop individualized treatment plans based on clinical findings, patient goals, and stroke best practices.
- Provide evidence鈥慴ased therapy for language, motor speech, cognitive鈥慶ommunication, and swallowing disorders.
- Monitor patient progress, revise care plans, and determine readiness for discharge or transition to rehabilitation.
- Recommend diet texture modifications and safe swallowing strategies.
- Provide communication strategies and supports for patients, families, and the care team.
- Contribute to discharge planning, including outpatient referrals or rehabilitation pathways.
- Participate in daily interprofessional rounds on NVU.
- Collaborate with NVU, general internal medicine, nursing, rehab teams, and patient flow partners.
- Communicate assessment findings and care plans clearly to teams, patients, and families.
- Maintain thorough, accurate, and timely clinical documentation.
- Assist with coverage across other Allied Health clinical areas as needed, including rotating weekend shifts.
Stroke Outreach (0.5 FTE)
- Provide SLP consultations for patients seen in:
- Stroke Prevention Clinic (SPC)
- TIA and Minor Stroke Unit (TAMS)
- Assess communication, cognitive鈥慶ommunication, and swallowing concerns in ambulatory/rapid鈥憆esponse settings.
- Offer short鈥憈erm, targeted education or intervention for mild communication or swallowing issues as needed.
- Identify patients requiring further outpatient or community SLP services and facilitate appropriate referrals.
- Support delivery of stroke patient and family education, including communication after stroke and swallowing safety.
- Participate in initiative development to enhance stroke care across the Toronto Stroke Network.
- Contribute to implementing stroke best practice guidelines and promoting evidence鈥慴ased stroke care.
- Participate in quality initiatives aimed at improving patient outcomes on the Neurovascular Unit and across the stroke system.
- Support stroke best practice implementation and contribute to stroke system priorities.
- Provide education sessions, in鈥憇ervices, or training for interprofessional staff and learners.
- Stay current with evidence-based practices in acute stroke and neurovascular SLP care.
- Practice in full accordance with CASLPO requirements and the Regulated Health Professions Act (1991).
