Virtual Behavioural Medicine is part of the Seniors Mental Health division at UHN and is a partnership with Baycrest hospital. VBM provides virtual assessment and care for older adults with dementia and behavioural and psychological symptoms within long-term care (LTC), complex continuing care, and acute care settings across the GTA and Ontario. This service is integrated with our Specialized Dementia Unit, a psychogeriatric unit caring for people with dementia whose behaviours are placing themselves or others at risk. VBM is a multidisciplinary team including nurse practitioners, geriatric psychiatry, social work, and pharmacy.
POSITION:
The Behaviour Support Transition Lead (BSTL) and Clinical Coordinator is an integral member of the Virtual Behavioural Medicine (VBM) team. Through the provision of patient and non-patient care activities, the BSTL/Clinical Coordinator enhances the quality of life of patients with dementia by: providing behavioural assessment; developing and coordinating patient care plans during admissions and discharges from the Specialized Dementia Unit (SDU); support communication between the VBM and SDU staff to ensure coordination of care; act as a liaison between Baycrest and SDU; supporting the development and implementation of program policies/procedures; improving clinic workflows; implementing organizational initiatives regarding quality improvements and program development; planning, organizing, directing, coordinating clinical activities related to patient care; monitoring program quality and performance; and other tasks as appropriate. The BSTL/Clinical Coordinator collaborates with the inter-professional team to provide patient focused care; communicates effectively with patients, families, and team members; participates in program planning, development and professional and hospital initiatives that support quality management and research. The BSTL/Clinical Coordinator complies with and shows commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures to ensure a safe environment for patients.
