The Care Coordination Team Manager is a serving leader who is a clinical and operational link between clinical care strategy and the Clinical Care Coordinators, ensuring that there is clinical standardization and programmatic consistency throughout all locations. Interfaces with clinical care coordination team regularly and serves as the point of contact for Care Coordination activities. This position works with multiple entities to include nursing, market and network services, payer contacts, information technology, primary care and specialty care leadership, enterprise care coordination and other disciplines, as appropriate. Functions as a clinical care coordinator in coverage situations and ensures best practices across the program.

A caregiver who excels in this role will:
MonitorCare Coordination team's core competencies, clinical andutilizationoutcomes, qualitymetricsandoutreach compliance.
Serve as the liaison between various payer programs, clinical care practices, market and network servicesand the care coordination teams.
Act as the key care coordination information and education resource for the interdisciplinary care team.
Maintain procedure manual, communicatechangesandcollaborate with clinical leadership on protocol review and changes.
Ensure workflow alignment with enterprise care coordination andpayer’squality and programmatic agreements.
Ensure care coordinatoridentifies, assesses,educatesand coordinates care for the high and rising risk patientsidentifiedin the clinical setting, focusing on patient outreach and the goal of achievingoptimaloutcomes.
Ensureappropriate distributionof patient load and effective management of patientpanel.
Provide leadership in hiring andonboarding ofnew nurses in collaboration with respectivepracticeand interdisciplinaryteam.
Serve as a coach and mentor for all clinical care coordinators and contribute to andcompleteAPRs.
Minimum qualifications for the ideal future caregiver include:
Bachelor’sof Science in Nursing (BSN)
Current state licensure as a Registered Nurse (RN)
Five years of nursing experience and one year Care Coordination or Case Management experience
Supervisory or management experience
Basic Cardiac Life Support (BLS)
Preferred qualifications for the ideal future caregiver include:
Master’sof Science in Nursing (MSN) inHealthcareor a related field
