You’ll be a key link between our clients, their families, Care Professionals and the office team. Your role will include:
- Carrying out client reviews and quality assurance visits.
- Conducting Care Professional supervisions, providing support and mentoring.
- Supporting care consultations and creating, reviewing and auditing client care plans.
- Carrying out client and Care Professional introductions.
- Maintaining regular contact with clients, families and Care Professionals.
- Supporting Care Professionals with digital care planning and care management technology.
- Ensuring client and Care Professional records are accurate and up to date.
- Building positive relationships with clients, families and other professionals involved in their care.
- Supporting the wider Client Experience team.
- Participating in the on-call rota when required.
