A Day in the Life
Assures spiritual assessment of patients/families/caregivers in the Hospice program and that appropriate services are rendered in a timely manner.
Facilitates the development in the interdisciplinary team plan of care to meet identified spiritual needs; works in close collaboration with local clergy when desired by patient.
Provides direct spiritual care to patients/families/caregivers when requested in accordance with the ongoing needs and wishes of the patient/family. Assess response to and satisfaction with care.
Serves as liaison and support to community chaplains and spiritual counselors.
Documents direct services and ongoing communication with community chaplains and spiritual counselors in the clinical record.
Attends patient care conference and interdisciplinary team meetings as a core member of the interdisciplinary team. Assists the team developing and updating the Plan of Care.
