Job Description:
Case managers serve as the care coordinators for patients, care givers, and families. Their functions include concurrent and retrospective review toĀ ensureĀ appropriate utilization, assessment, prevention ofĀ denials,Ā and coordination of discharge options. TheyĀ are required toĀ follow specific criteria sets toĀ ensureĀ regulatory compliance. They coordinate safe discharge plans between the health care delivery team, patients, families,Ā insurers,Ā and continuing care agencies.Ā
Education/Experience:Ā Ā
Ā Graduate of an accredited school of nursingĀ
Ā BSN preferredĀ
Certification/License:Ā
Current Massachusetts RN licensureĀ
Ā Certification in Case Management preferredĀ

Pay Range:
$74,880.00 USD - $164,571.21 USDThis position is covered by a collective bargaining agreement.Ā The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time.Ā The pay range for this position is established and governed by the terms ofĀ the applicable collective bargainingĀ agreement. Employees hired into this position will receive compensation consistent with the terms of the applicable collective bargaining agreement.Ā
