The Oncology Care Coordinatorin this rolewillbe responsible forthecoordination of multidisciplinary clinic programs and care continuum foroncology patients going throughtheir journey with cancer.You willhelpnavigatethesepatientsthroughpre- and post-operative care as well as through needed therapies.This is an excellent opportunity for an experienced nurse who has a passion for oncology and wants to branch out in their nursing career.
A caregiver in this position worksdaysfrom 8:00AM –4:30PM.
A caregiver who excels in this role will:
Coordinate multidisciplinary clinic programs, programdevelopmentand services.
Navigate the care continuum for patients by collaborating with the Medical Director and act as a "compass" for patients diagnosed and/or receiving therapy.
Ensuretimelyscheduling of diagnostic work-up,resultsreporting and communications between patients and the multidisciplinary team.
Promote the multidisciplinary team process andensurepatientreceivesnecessary referrals and follow-up.
Provide efficient and effective care for patients and families and keep current with emerging technologies and treatment trends.
Coordinate cancer relatedpatient/staff education programs, projects,servicesand groups.
Assistwith community events, evaluate theresultsand continueoutcomesresearch.
Compile and analyze Quality Improvement data and benchmarking.
Serve as a patient advocate by promotingtimely,competentand compassionate care.
Complete mandatory education and training tomaintainorganization anddepartmentspecific competencies and requirements and certification/licensure.
Minimum qualifications for the ideal future caregiver include:
Program completion in professional nursing
Current licensure as a Registered Nurse (RN) in state of employment
Basic Life Support (BLS) Certificationthrough the American Heart Association (AHA) or American Red Cross
Any registered nurse or advanced practice nurse must obtain a cancer-specific certification ordemonstrateongoing qualifying education within thetimeframeof the facilities accreditation cycle
Knowledge of oncology prevention diagnosis disease process and symptom management
Five years of nursing experience including two years in oncology
Preferred qualifications for the ideal future caregiver include:
Bachelor’sof Science in Nursing (BSN)
Experience in case management mental health program development and community health/home care
Oncology Certified Nurse (OCN)
Oncology experience
Personal Protective Equipment:
