We are looking for an experienced, driven Physical Therapist to join our team in a dual patient care and leadership capacity. This is a leadership opportunity for a clinician who is passionate about quality care and wants to help shape the clinical standards of a growing practice.
Patient Care (Clinical)
Evaluate, diagnose, and treat patients in their homes across New York City for current and developing partnerships
Develop individualized care plans and progress patients toward functional goals
Maintain accurate, timely documentation and billing in compliance with payer and regulatory requirements
Communicate effectively with physicians, caregivers, and other members of the care team
Clinical Oversight (Lead PT Responsibilities)
Serve as a first point of contact for contract PTs with clinical questions and case-related guidance
Conduct regular documentation/note audits to ensure quality, accuracy, and compliance
Proactively address performance gaps among contract PTs and perform timely, professional outreach to address concerns
Community Outreach & Business Development
Visit local physician offices, specialists, and referral sources to drop off practice collateral and nurture relationships for current and developing partnerships
Serve as an ambassador for our brand and care model within the provider community
Required
Doctor of Physical Therapy (DPT) degree from an accredited program
Active, unrestricted New York State Physical Therapist license
Minimum 3 years of clinical PT experience
Strong documentation skills with experience in EMR/EHR systems
Ability to travel throughout New York City boroughs via public transit, exact treatment location to be discussed
Preferred
Prior experience in a clinical lead, supervisor, or mentorship role
Familiarity with Medicare and private payer documentation requirements
