Cleveland Clinic's Heart, Vascular, and Thoracic Institute (HVTI) isseekinga full-time experienced, dedicated and innovative Advanced Practice Provider (APP) to lead and develop strategic initiatives within the HVTI Community Outreach Program.
This role is designed to focus on cardiovascular disease (CVD) prevention, early detection, patient navigation, and clinical trial enrollment, while fostering relationships with community stakeholders and internal Cleveland Clinic partners. The APP will play a pivotal role in addressing disparities in healthcare access and outcomes, particularly in underserved urban and rural communities.Ā
Key Responsibilities:
1.Attend Community Screening Events- Attend and activelyparticipatein community screening events, partnering with local organizations.
2. PatientNavigation:Review screening results and ensuretimelyfollow-up on abnormal findings. Order follow-up diagnostic testing and specialty consultations.
3. Community-BasedClinics:Outpatientcardiovascularclinic at Stephanie Tubbs Jones Health Center, Euclid Hospital and/orLangston Hughes Community Health Center.
Ā 4. Education:Develop and lead APPHeart Failure(HF) education group visits at community sites to improve population health outcomes.Establishan APP educational curriculum focused on disease states, preventive care, and referral processes to enhance community health literacy.Ā
5. Clinical Trial Awareness andEnrollment:Promote awareness of HVTI clinical trials during community events, focusing on arrhythmia, TAVR/structural heart disease, and other HVTI-related trials.Identifyand enroll eligible patientsinclinical trials to advance research and improve access tocutting-edgetreatments.Ā
6. Grant Writing and ResourceDevelopment:Identifyand pursue grant opportunities to secure funding for community outreach programs

*Minimum ofĀ 2 yearsĀ outpatient clinical APP experience isĀ required.Ā PreviousĀ cardiovascular and/or primary care experience is preferred. Functions as an APRN/PA in collaboration with a Physician.
Flexible schedule.Ā Some evenings/weekends may be required to attend screening events.
Nationally certified Advanced Practice Provider who provides primary and/or specialty care in a variety of healthcare settings. Functions in collaboration with the health care team in accordance with certification and licensure. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Responsible for dissemination of knowledge through instructing, leading and developing educational programs.

Graduate of an A.R.C. approved Physician Assistant Program.Ā
Masterās degree in Physician Assistant Studies and/or higher degree obtained from a program recognized by their practicing states Licensing Board.Ā

Currently licensed as a Nurse Practitioner in applicable state(s).Ā
Must be licensed as an RN in applicable state(s).
National certification as a Nurse Practitioner by nationally accredited organization recognized by state Board of Nursing.Ā
Basic Life Support (BLS) through the American Heart Association (AHA) required.Ā
Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) through the American Heart Association (AHA); or Neonatal Resuscitation Program (NRP) through the American Academy of Pediatrics (AAP) may be required based upon work setting.Ā
