Job Summary
The Lead, Referral System & Inter-Care Coordination is responsible for managing, optimising, and standardising patient referral pathways across all healthcare facilities. The role ensures seamless coordination between primary, secondary, and tertiary care services, improving patient access, continuity of care, and overall health system efficiency. This position develops referral protocols, monitors referral performance, and provides guidance to staff to strengthen inter-facility collaboration and patient-centred service delivery.
Responsibilities
- Lead the design, implementation, and monitoring of referral pathways and inter-facility care coordination processes across healthcare facilities.
- Develop, standardise, and update referral policies, protocols, and documentation to ensure efficient and safe patient transfers.
- Monitor referral data and performance indicators to identify bottlenecks, inefficiencies, and opportunities for improvement.
- Collaborate with clinical teams, facility managers, and administrative staff to ensure timely and appropriate patient referrals.
- Provide training, technical guidance, and capacity-building support to facility staff on referral processes and coordination best practices
- Analyse referral trends, patient flow data, and outcomes to inform system improvements and strategic decision-making.
- Ensure alignment of referral practices with national healthcare standards, organisational policies, and quality improvement initiatives.
