**This role is a full time outpatient position at 40 hours per week. The schedule is Monday to Friday, 8-hour shifts and is supporting NM Family medicine. Role may require coverage at different locations as well as virtually. **
The Advanced Clinical Dietitian reflects the mission, vision, and values of NM, adheres to the organization鈥檚 Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Advanced Clinical Dietician, in collaboration with the physician, provides nutritional counseling in order to restore or maintain an optimal nutrition status for the patient. This position communicates and collaborates effectively with the health care team to exchange and disseminate information regarding care of the patient and program development.
Responsibilities:
- Collaborates with physician to identify and connect with patients in need of nutrition counseling.
- Evaluates patient and nutrition parameters, identifies goals/outcomes, completes risk assessment.
- Selects appropriate dietary, behavioral, and physical interventions.
- Provides nutrition/lifestyle education and counseling for individuals and groups.
- Discusses/interprets appropriate use of pharmacotherapy, makes recommendations as appropriate (supplements, commercial products).
- Develops, presents, and modifies educational materials.
- Evaluates effects of nutrition counseling, observes and notes patient鈥檚 response and progress, recommends adjustments and modifications.
- Consults with other programs/staff to ensure coordinated service delivery.
- Identifies and communicates key issues and trends to health care team.
- Participates in in-service training for staff, students, and physicians.
- Educates target groups about the role of nutrition in disease management and lifestyle medicine.
- Collects and documents research data to support quality management outcomes.
- May perform other duties as assigned.
