The Pharmacy Population Health Coordinator supports clinical pharmacy programs within the Beth Israel Lahey Health Performance Network (BILHPN) ā Clinical Excellence Division. The role focuses on improving medication adherence, closing care gaps, and enabling performance in our valueābased payer contracts.Job Description:
Essential Duties & Responsibilities including but not limited to:Ā
ā¢ Review medication adherence, quality, and performance reports to identify patients needing intervention.
ā¢ Conduct outreach to patients, providers, pharmacies, and care teams to address adherence gaps, medication access issues, and pharmacy benefit questions.
ā¢ Assist with addressing barriers to medication access including coordination of refills and prescription assistance programs.Ā
ā¢ Route prioritized intervention opportunities to clinical pharmacists based on workflow and risk criteria.
ā¢ Support pharmacy-led population health and quality improvement initiatives.
ā¢ Perform administrative and operational tasks allowing clinical pharmacists to practice at top-of-license.
ā¢ Document all activities in the electronic medical record per established workflow.
ā¢ Participate in program data tracking, reporting, workflow optimization, and resource development.
ā¢ Participate in the education of PGY1 and PGY2 pharmacy residents and pharmacy students where applicable.Ā

Minimum Qualifications:
Education:
High school graduate with college degree preferred.
Licensure, Certification & Registration:
Certified Pharmacy Technician in Massachusetts; willing to obtain additional state licensure as needed.
Ā
Experience:
2 to 5 years of experience in retail pharmacy, managed care, or population health environment.
Skills, Knowledge & Abilities:
ā¢ Strong communication, time management, and organizational skills.
ā¢ Ability to work independently within scope of work.
ā¢ Experience with medication adherence, valueābased care contracts, and motivational interviewing skillsĀ preferred.
ā¢ Ability to communicate effectively with healthcare professionals and patients in person andĀ telephonically.
ā¢ Ability to research and analyze medical literature, drug information, evidence-based guidelines andĀ pharmacy, and cost utilization data.
ā¢ Knowledge of HEDIS, Stars Quality metrics preferred.
ā¢ Knowledge of Massachusetts / national Prescription Assistance Programs.
ā¢ MS Office applications including Word, PowerPoint, Excel and Outlook required; Experience withĀ databases preferred.
ā¢ Experience with Epic Electronic Medical Record preferred.
Pay Range:
$27.01 - $36.35The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geographyĀ of work location, job responsibilities, or other applicable factors permissible by law.Ā Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
